Payment Options
- Fees are based on the fee schedule you and I select, that fits your healthcare needs and budget.
- Packages can be paid by Personal Check, Cash, Visa or Master Card.
- A package paid in full must be used within one year and is non-transferable.
- All herbs and other products are charged separately.
Medical Information
- In order for us to give you the best treatment we need to have all of your medical information.
- Please be accurate in filing out your medical history and advise of any changes in treatment, medication, herbs or supplements.
- All medical records will be kept confidentially and in compliance with HIPPA
From time to time, we burn an herbal preparation of Mugwort called Moxa to warm acupuncture points (it smells like burning leaves), and light candles for aromatherapy and relaxation. If these are irritating or offensive, let us know and we will not use them during your treatment.
